In the context of this document, I request that my representative(s) note the following:

I do not have any religious convictions concerning the sanctity of life
I do not have any objection to euthanasia
I do not believe in 'settling' for any significant diminution in quality of life
I wish to donate my organs and body to medical science
I do not require any form of burial or cremation

I believe the following situations are incompatible with quality of life and life in acordance with my values:

Loss of cognitive function to a level significantly below that of the majority of the population
Significant physical disfigurement to an extent likely to cause comment in public (such as from burns or surgery)
Loss of one or more limbs
Permanent loss of vision so as to prevent unassisted use of a computer, television, or reading matter
Continuing medical treatment which would require me to remain in the same country for more than 6 months (for example dialysis or chemotherapy)

For 45 of the preceding 90 days and for the preceding 7 days:
Inability or unwillingness to attend unassisted to personal hygiene (washing, toileting)
Inability or unwillingness to communicate verbally to a level present in a the majority of the population
Inability or unwillingness to consume nutrition unassisted
Inability to walk more than 500metres unassisted (other than with a walking stick)
Profound loss of or distortion to taste
Profound loss of hearing
